Hours of Operation are Monday through Friday, 8 AM - 6 PM
We request the courtesy of a 24-hour notice when cancellation of an appointment
is required. We will make every effort to accommodate your schedule.
- For your first appointment, we ask that you arrive 5-10 minutes early to fill out
paperwork.
- Please wear comfortable clothes that are appropriate to your injury. For example:
shorts, sneakers, and t-shirts.
- Also, please remember to bring your insurance card, a form of identification, and
your physical therapist prescription from your referring physician.
